Prior to this appointment, she served as ProMedica’s corporate director of advocacy and community impact. Kate is a graduate of Purdue University and earned a Master of Public Administration degree at Bowling Green State University.

Brian’s teams provide the organization with corporate and line of business intelligence and measurement. His teams also lead efforts related to Priority Health’s member portals and transparency tools.

In his work, Brian’s goal is to simplify the complex world of health care through the use of data and analytics to engage members in their health care decisions.

Brian has more than 15 years of experience in health care and 30 years of experience in management roles. He holds a Master of Business Administration from Northwood University and a Bachelor of History from Michigan State University.

Ashley Perry, MPH

Ashley manages a multidisciplinary team that translates Social Risk Intelligence™ into quantifiable business results and community impact. As a public health practitioner, researcher, and management consultant, she has bridged healthcare and public health. Prior to Socially Determined, she built and scaled successful consulting practices for the Premier Healthcare Alliance and Kaufman Hall. Ashley also previously led research and programmatic initiatives for organizations ranging from an academic research center to a network of federally qualified health centers.

She holds a Bachelor’s degree in Economics from Duke University, a Master’s degree in Public Health from Columbia University and is a certified Public Health professional. In 2020, Ashley was recognized by Modern Healthcare as a Top 25 Emerging Leader and “one of the most impressive leaders driving change across the industry.”

Andrea Zaldivar, Ed. D, M.S., ANP-BC

Andrea is responsible for researching, planning, organizing, and implementing relationships and activities that support MetroPlusHealth members in maximizing their health outcomes through improving social determinants of health status. These determinants include but are not limited to housing stability, food insecurity, social engagement, health literacy, training, education, retained employment, and access to health care.

Andrea has served on many national and local boards and professional committees, including the American Diabetes Association (ADA) and the Center for Disease Control (CDC) – Division of Diabetes Translation – Latino Workgroup. She has published and co-authored articles in The Diabetes Educator and Home Healthcare Now and a chapter in the Complete Nurse’s Guide to Diabetes Care.

Andrea received her Ed. D – Health and Behavior Education from Teachers College Columbia, where she was a fellow in Research and Health Disparities and an M.S. – Adult Nurse Practitioner from Columbia University. Her B.S.N – Nursing was from Seton Hall University. Additionally, she completed the Population Health Coordinator Program (PCCP) at Duke University. Her special interest is in addressing health disparities in chronic disease delivery, especially in Diabetes care in Latino elder adults.

Omolara Thomas Uwemedimo, MD, MPH

Omolara is a business development and funding coach for women of color in healthcare and a serial entrepreneur, growing 2 companies to multi-six figures in revenue in less than 18 months. During her intrapreneurial career as a pediatrician for over 15 years, researcher & professor for over a decade, she secured $2 million in grant funding and has led inter-professional teams to build and scale healthcare delivery and research programs to achieve health equity for marginalized youth and families. Her work has been defined by a passion for social justice, serving as an advocate and working as a global physician across sub-Saharan Africa, Asia, and the Caribbean.

In 2019, Omolara co-founded Strong Children Wellness, a multi-award-winning, healthcare practice network in NYC, providing integrated physical health, mental health and social services for minoritized, low-income children and families. As CEO, Dr. Uwemedimo and her co-founders have been able to sustainably grow during the COVID-19 pandemic, raising over $600k in grants, impact investment awards and contracts. Her medical practice has been featured in the New York Times, NY1 News and Crain’s for their reverse integration model, embedding care within community organizations including nonprofits and foster care agencies.

Given her expertise and success as an entrepreneur, Dr. Omolara went on to found Melanin & Medicine, premier business development & funding coaching company to help support women of color in medicine to secure capital, without incurring debt or diluting equity, to build and grow their own healthcare businesses committed to social impact. To date, Melanin & Medicine has helped over 200 women in medicine across our workshops, courses and programs to pivot into their purpose, re-create their healthcare careers on their own terms, build social impact businesses and live more fulfilled & integrated lives. In addition, they support thousands more through their popular Melanin & Medicine weekly podcast.

Omolara is an expert in purpose-led entrepreneurship, securing funding for healthcare, addressing medical racism for marginalized providers and patients and building socially-responsive healthcare spaces. Her work has been featured in several media outlets including People.com, NBC News, Medscape, Essence.com, Newsweek, Crain’s, Politico, Reuters, and NPR.
