Cynthia partners with physician leadership to ensure success in all models of managed care, accountable care and value-based contracting, including episodic payment models. Since 2006, Cynthia has been operationally responsible for programs and services focused on care coordination, high risk patient management, disease prevention and management, utilization management, clinical integration, quality management and clinical performance improvement.

She oversees Cedars-Sinai’s multi-specialty IPA, its Medical Provider Network for Workers’ Compensation, and five commercial Accountable Care contracts. Cynthia served previously as the Vice President for Medical Network Development, the Executive Director of Operations and the Director of Business Development for Cedars-Sinai Medical Care Foundation, and the Director of Administrative Affairs for Cedars-Sinai Medical Center. In these various roles, Cynthia oversaw physician recruitment and practice valuations, EMR implementation, and medical group operations.

She has also served as a Service Line Director for Cedars-Sinai’s Department of Pediatrics and Medical Genetics. Cynthia has been with Cedars-Sinai since 1990, with the exception of a brief hiatus in 1998-99 during which she served as the Administrator for Women’s Health at the University of California San Francisco.

Cynthia received her BS from Cornell University and her MPH from Columbia University. She lives in Tarzana, California with her husband and two children.

Jessica Hohman, MD, MSc

Jessica Hohman, MD, MSc, is a primary care physician in the Department of Internal Medicine and currently the President and Medical Director of the Cleveland Clinic Accountable Care Organization. In this capacity, Dr. Hohman oversees 90,000 beneficiaries, with a particular focus on designing interventions to improve healthcare quality and value. She has led a portfolio of population health initiatives—including development of a data-driven utilization management program, creation of a high-intensity home-based rehabilitation models, and implementation of collaborative approaches to improve end-of-life care.

Dr. Hohman is also a physician investigator in the Center for Value-Based Care Research. She has presented at national meetings and co-authored publications on health IT, telemedicine, healthcare financing, and post-acute care. She maintains an active clinical practice in Internal Medicine, seeing both outpatients and inpatients and is involved in the teaching and mentorship of Case Western and Cleveland Clinic Lerner School of Medicine students and residents.

Lissy Hu, M.D., MBA

Dr. Lissy Hu serves as president of CarePort, powered by WellSky’s connected networks, where she works with leading health systems, payers, and provider organizations to optimize post-acute care outcomes. Lissy brings with her over 15 years of experience working in the healthcare technology space. In 2012, Lissy founded CarePort to provide care coordination software solutions to manage patient transitions across the continuum. CarePort’s end-to-end platform bridges acute and post-acute EHRs — providing visibility for providers, payers, and risk-bearing organizations into the care that patients receive across care settings. Lissy earned her Doctor of Medicine from Harvard Medical School. She holds a Master of Business Administration degree from Harvard Business School and holds a Bachelor of Arts degree in pre-medical studies and sociology from Columbia University.

Margie Zeglen, MBA, RHIA

Since 1996, Margie Zeglen has provided progressive healthcare leadership experience. She serves as vice president of Population Health where she oversees population health services, care management, post-acute collaborative and risk adjustment solutions. Her focus is on value based care models and leading the work associated with advancing outcomes, quality, safety and health of the population.

Ms. Zeglen has experience with large-scale system change, operational transformation and care redesign initiatives focused on ensuring quality, operational performance and integrated care. Her prior responsibilities have included oversight for utilization management, bundled payment programs, a clinically integrated physician network and Accountable Care Organization (ACO) participation. She has had responsibility for home care and palliative care services and has successfully developed Post-Acute Care collaboratives.

Margie also has firsthand experience with risk adjustment, data analytics and provider performance reporting. Her passion is making data actionable and meaningful on first impression. Her practical approaches to education, engagement including innovation technology and actionable analytics support adoption of fluid reimbursement models that favor a value-based system.

Ms. Zeglen holds a degree in Health Information Management, a Master’s of Business Administration with concentrations in management of innovation and technology and organizational leadership. She is also a Fellow of the American College of Healthcare Executives (ACHE) and serves on the National AHIMA Health IT and Innovation Practice Council. Ms. Zeglen has contributed to local and national publications and presented at national conferences.
