How to Create More Bright Spots in Post-Acute and Home Care

Rob Schreiber, M.D is the Vice-President and National Medical Director of myPlace Health of Long Beach, California, a risk-bearing care delivery platform, that will deliver services to vulnerable older adults through the PACE (Program for All-Inclusive Care for the Elderly) model and as a value-based provider working in partnership with local health plans and provider groups.

Prior to this, Dr. Schreiber served as Vice President and Medical Director of Summit ElderCare, one of the largest Program of All-inclusive Care for the Elderly (PACE) in the United States and had served in this role for five years. He was the chair of the National Pace Associations’ Medical Directors Subcommittee for two years prior to his new role and has given multiple national presentations involving PACE innovation over the past four years. He is a featured speaker at the annual symposium and has worked to scale PACE nationally as a preferred model of integrated care for dual eligible nursing home eligible older adults and those who have only Medicaid or Medicare.

From 2012-2017, Dr. Schreiber served as Medical Director of Evidence based Programs at Hebrew SeniorLife and Medical Director of the Healthy Living Center of Excellence (HLCE), an organization funded by the John A. Hartford Foundation and the Tufts Health Plan Foundation. The Center strives to promote the independence of older adults by empowering them and their families to take more active roles in their care by offering quality, proven interventions addressing issues including, but not limited to management of chronic illness, methods to prevent and manage fall and promote physical activity and proper nutrition. This is being achieved through the implementation of evidence- based disease management programs to empower older adults to actively manage their conditions. Programs, including the Stanford University Chronic Disease Self-Management Programs, focus on multiple behavior changes to achieve better health.

Dr. Schreiber also served as a geriatric consultant in a Patient Centered Medical Home for an Accountable Care Organization to develop a systems approach to managing the high-risk older adult population. He developed a comanagment model of complex care management that is now being replicated in this large ACO, his practice focused on patients with cognitive impairment and dementia, palliative care, and multiple morbidities.

Dr. Schreiber has published on improving skilled nursing facility care, avoidable hospital readmissions, palliative care in the nursing home leading to better quality of life and longevity, transitions of care using Project Red and improving quality of care institutionally through a transformative team improvement approach in skilled nursing care facilities. He most recently facilitated the John A. He has had the opportunity to be part of the PCORI Initiative Patient Priority Program, now known as Patient Priority Care, being led by Mary Tinetti MD. He served on the Research Advisory Panel to help determine the types of research questions that need to be studied in this new transformational model of primary care. He has been elected as a Fellow of the American Geriatrics Society and has served on multiple committees for this organization over the past 20 years. He presently serves as a member of the Public Policy Commission

Dr, Schreiber served as Physician-in-Chief and CMO of Hebrew SeniorLife in Boston MA from 2004 to 2012. He helped to develop the strategic direction of Hebrew SeniorLife (HSL) Medical Group in home and community-based services, long-term care and post acute care. He has been a faculty member of the Institute for Aging Research at Hebrew SeniorLife and is working to connect research to health care systems clinical venues. He serves on the faculty at Harvard Medical School from 1996 to present and was a Senior Leader of the Practice Change Leaders Program which mentors’ physicians, nurses and social workers improve the care of older adults in their health systems.

Prior to Hebrew SeniorLife, he served as the Department Chairman at Lahey Clinic in Burlington, Massachusetts where he developed innovative geriatric evaluation and management units in skilled nursing facilities that resulted in improved population health outcomes at lower costs. In addition, he led the development and implementation of a care management program that was highly successful in managing the most complex older adults in a globally capitated Medicare HMO program from 1999 to 2004.

Dr. Schreiber has also served as a national consultant for the National 4A National Aging and Disability Institute which is helping community-based organizations develop the business acumen to engage with health care providers, healthcare systems and insurers. He was awarded the American Medical Director’s Physician of the Year Award for 2010.
