Launched in 2014, Landmark currently is in 25 states with around 300,000 patients. By the end of next year, the integrated health provider will be in about 40 states with just over half a million patients.

As a fully mobile medical group, Landmark brings medical care into the homes of seniors and chronically ill patients. These home-based visits are conducted by doctors, advanced practitioners, and care team members to supplement the care patients receive from their primary care providers and specialists – at no additional cost.
